Treatment adherence in users of mental health home hospitalization (HDOM)

The home hospitalization programme of the Consorci Corporació Sanitària Parc Taulí de Sabadell is a care service for admission in the person’s own home for people with a mental disorder in a phase of acute decompensation, requiring intensive care by psychiatry and nursing with daily family supervision. This study sets out three main objectives: a) to analyse insight capacity through a self-assessment of health status by the user and compare it with the HoNOS (Health of the Nation Outcome Scales) scale, b) to determine the level of treatment adherence according to sex and diagnosis, and c) to analyse the various reasons related to treatment non-adherence, observing whether there are differences according to diagnosis, age and sex. The sample comprises 30 individuals admitted to home hospitalization between December 2018 and March 2019. A self-assessment at admission, the HoNOS scale and the Morisky-Green test were carried out. In the analysis between the ‘self-assessment’ and ‘HoNOS’ variables at admission, there is a moderate positive correlation (Pearson correlation coefficient [r] = 0.538). According to the Morisky-Green test, only 30% of users are considered adherent. The results reflect that the main reason for non-adherence is forgetfulness, followed by the appearance of adverse effects and difficulty in preparing and administering the treatment. The results obtained support the use of the pill organizer as one of the simplest means on which we can act.
